Your Summer Discovery Signed Waivers Have Been Received

You will receive a link to a portal to upload a current physical for your child. Please click here to obtain a copy of the document that we need your child’s doctor to fill out.

Your application is only considered final once we receive your child’s physical. Your child cannot participate in Summer Discovery if we do not have the physical form filled out. Physicals must be submitted by June 17.
